The first figure shows the fat as yellow colour and light blue is the water jet being injected in the fat. The dark blue and red trees like structure are the blood vessels and the nerve. In the 2nd figure see how the water jet separates and dislodges the fat, which is then sucked out in the 3rd figure. In the last figure you can see the fat has been sucked out but the blood vessels and the nerves are left intact.

This is the special needle, which at its tip is spraying the water jet (blue) and the holes behind the tip of needle is simultaneously sucking out the fat (represented by the yellow dots).
